Hey everyone. A classmate and I are putting together an informal research paper on the effects of drugs on the human body and mind for our science class. We would really appreciate if you considered taking 5 minutes of your time to answer this quick survey regarding the topic whether you are a user, an ex-addict or nonaddict participating on behalf of a loved one.

No personal data unrelated to drug consumption is asked of you aside from age, which is required to be 18 or above because of the legal implications of the issue at hand. We tried making each question as impersonal, general and non-invasive as possible, although if you are nonetheless uncomfortable providing an answer to any of the questions we ask that you cease taking the survey. There’s an extra question at the end and a text field in case you would like to add a comment or anecdote relating to your experiences with drugs, although it is entirely optional. We are aware that this is a sensible matter and we will try our best to handle the data we amass with the utmost diligence and responsibility.
